PHOENIX (AZ Family) — Each week, Arizona Family investigates restaurants recently inspected by the Maricopa County Department of Environmental Services. Restaurants with the highest number of “risk factors” deemed by inspectors to be significant health code violations were selected for our Dirty Food Report. Here are the Valley restaurants that didn’t make the grade on November 13, 2024:

India’s Guru Palace – 2048 E Baseline Road, Mesa

3 violations

• Cooked chicken kept past its throw away date
• Bag of chopped cheese that has not been kept cold enough
• Baked potatoes are not at the appropriate temperature

Devil’s Advocate Bar and Grill – 955 E University Way, Tempe

3 violations

• No certified food manager on site
• No paper towels in the sink
• Orange organic material in soda guns

dog house – 2780 W Chandler Boulevard, Chandler

3 violations

• Employee not washing hands properly
• Employee processes raw beef patties and then bakes buns without washing his hands
• Raw egg bowl over sauces and sauces

Black Bear Restaurant – 2805 S Alma School Rd, Chandler

3 violations

• Employee took a raw egg, then took toast
• Raw beef stored over cooked ham
• Organic food scraps in slicer

Tampopo Ramen – 3223 S McClintock Dr, Tempe

4 violations

• Several employees wash their hands without soap
• Employee washed dirty dishes but then did not wash his hands
• Cooked pork and corn not kept cold enough
• Hand sanitizer stored directly above spices
